For students who would like to enroll early online, please accomplish this form. By submitting this form, the student is certifying that all information contained are correct and that it is the intention of the student to enroll at Centro Colegio de Tarlac. After submission of this form, the Registrar is hereby authorized to enter the student's name in the school's LIS. PLEASE DO NOT SUBMIT THIS FORM IF YOU DO NOT INTEND TO ENROLL.
